The Salvation Army Midland Division Veteran’s Residence is looking for a Transitional In Place Case Manager to supervise and coordinate engagement in assistance veterans with transitional in place support.
This is a full-time, 40 hours per week position with an annual salary of $50,000.
Location: 2935 Locust, Saint Louis, Missouri
Essential Functions
Professionally complete case management functions such as intakes, assessments, service/care plans, progress notes, case coordination notes, incident reports, notices, discharge summary, and post discharge contacts within required timelines.
Complete all documentation, reports, etc. using professional language for the field of practice.
Complete home visits with Housing Specialist to ensure a safe environment.
Provide daily, weekly, monthly, quarterly, and bi‑annual outcome measurements and report progress to supervisor.
