Responsibilities

The Case Manager supports social assistance participants using a strong emphasis on empathetic, person-centered, strengths-based support. The role goes beyond administering benefits and determining ongoing eligibility for Ontario Works. The Case Manager works collaboratively with individuals to reduce barriers, support stability, and help people move toward their own goals by conducting comprehensive assessments, developing action plans, and coaching conversations while providing wrap-around supports.
