Time-Limited Subsidy (TLS) Case Manager
The TLS Case Manager provides housing-focused case management to individuals and families experiencing homelessness. This role supports rapid rehousing efforts, helps clients secure and maintain permanent housing, and ensures compliance with program and funding requirements.

Key Responsibilities:

+ Manage a caseload and provide strengths-based, housing-focused case management

+ Develop housing retention plans and conduct regular client follow-ups

+ Assist clients with housing applications, landlord coordination, and rent sustainability planning
