Job Description
Case Manager - Community Mental Health
* Registered Health Care Practitioner - Registered Nurse/Psychiatric Nurse, Occupational Therapist, Social Worker * Based at Kensington Centre 'Timaru Hospital * Monday to Friday - 8.30am - 5pm

About the Role
The primary purpose of the role is to deliver coordinated comprehensive, and person-centred case management for individuals with moderate to severe mental illness. This includes assessing needs, developing and implementing care plans, connecting service users with resources and monitoring progress to support recovery independence and improved quality of life.
