Position Summary:

Provide case management service to clients as outlined in the case management procedure manual in accordance program guidelines



Essential Job Functions:




+ Provide a minimum of 115 hours of service for assigned clients each month.

+ Complete client interview and submit written findings and recommendations to referral source

+ Coordinate the provision of mental health treatment, competency restoration training, residential care or housing with supervision, medical and auxiliary services if appropriate, case management and monitoring of individuals who are being diverted from commitment to the Department of Children and Families.
