Description

Position Summary:

Person in this position will be responsible for the case management of a caseload of people living with AIDS. Essential Job Functions include those listed below.

Essential Job Functions:


+ Outreach and Case management of PLWA addressing homelessness.

+ Link consumers to community-based providers to obtain supportive services.

+ Make follow up contacts with Community Partners and consumers in the program to insure a smooth continuum of care.

+ Documentation of all outreach efforts and linkages to care.

+ Coordination of services addressing all holistic approach to wellness and independent living.

+ Attend scheduled meetings within the community.

+ Travel to community-based providers.
