Job Description

To provide basic care coordination, discharge planning, monitor/evaluate and education for clients and their families on the services offered by NTUC Health Co-Operative Ltd.


You will need to:


(a) Information and Referral (I & R)

  • Manage various referral sources such as emails, phone calls and walk-ins in order to link clients to appropriate services by identifying and accessing their needs.

    • (b) Service Coordination

  • Administer the coordination of service requested by clients. To ensure proper documentation on the systems and paper filing are completed on a timely basis.

    • (c) Case Management

  • Conduct regular home visits and telephonic follow-ups on a monthly basis to engage and build rapport with clients and their caregivers, and obtain feedback on the services rendered.
