Description

The Care Coordinator for the Diversion Program plays a vital role in coordinating services for justice-involved individuals with behavioral health needs. This position is responsible for engaging participants, developing individualized service plans, and ensuring timely linkage to mental health, substance use, medical, housing, and social support services.
