This cardiology based role focuses primarily on the management of patients with congestive heart failure (CHF). The APP will provide timely post discharge follow up, provide treatment in alignment with national guidelines, and monitor laboratory and clinical data to support ongoing care. The position emphasizes longitudinal management with the goal of reducing readmissions, while also coordinating prior authorizations and serving as a clinical liaison with transplant centers as needed.
