The After Office Hours Care Manager is an integral member of the after-hours mental health program, responsible for coordinating and supporting patient-centered behavioral health care. This role partners closely with providers, patients, and families to monitor treatment progress, address barriers to care, and promote engagement, education, and adherence to treatment plans.
