The After Office Hours Care Manager is an integral member of the after-hours mental health program, responsible for coordinating and supporting patient-centered behavioral health care. This role partners closely with providers, patients, and families to monitor treatment progress, address barriers to care, and promote engagement, education, and adherence to treatment plans.

What You’ll Do


+ Coordinate and support mental health care services in collaboration with behavioral health providers and clinical staff

+ Monitor and track patient treatment responses, including changes in symptoms, side effects, and potential complications
