In this role, you will be responsible for delivering high-quality statutory social care, including safeguarding, assessment, supervision, and care management functions, as well as developing innovative community solutions. Advanced Practitioners are pivotal in developing links with the local community through multi-agency work with partner organisations, providers, professionals, individuals, families, and carers. You will ensure that all statutory health and social care responsibilities are met and delivered to a high standard.
