Overview:

Sentara is looking for a Part Time Hospice Advance Practice Provider for Chesapeake Hospice. The APP will be responsible for in home visits, telehealth appointments and returning patient phone calls. There will also be Teams and In person office meetings. This is a Hybrid position.

Hours- Any 3 days--Monday- Friday 8am-5pm (24 hours per week)

The Nurse Practitioner (NP) is responsible for the provision of health care services to individuals, families, and/or groups who receive their care under the auspices of Sentara. Health care services include conducting health assessments, prescribing/providing treatments/interventions, promoting health, and providing disease prevention and management.
