Job Description
**RN Case Manager – Addiction Services Program**
Location: Lynn, MA
Facility Type: Health Center / Clinic
Schedule: Monday–Friday, 8:30 AM – 5:00 PM (No weekends or holidays)
Duration: 13-week contract to start, with strong potential for extension
Pay Rate: $55/hour
**Position Overview**
The RN Case Manager plays a key role in coordinating comprehensive care for patients with substance use disorders. This position focuses on developing individualized care plans that address each patient’s medical, psychological, and social needs.
Working closely with a multidisciplinary team, the RN Case Manager ensures patients have access to necessary resources and services to support recovery and improve health outcomes. This role also emphasizes patient education, engagement, and long-term care coordination.
