The Licensed Marriage and Family Therapist (LMFT) works collaboratively with a multidisciplinary team to contribute to the development of care plans for patients/clients in need of social work services. The LMFT provides counseling and crisis intervention to individuals experiencing social or emotional challenges across a variety of settings and age groups.

**Minimum Requirements:**

+ Master’s degree in Social Work from a program accredited by the Council on Social Work Education required
+ Active LMFT license in the state of practice required
+ Minimum of one year of social work experience preferred
+ Current CPR certification, if applicable
+ TB screening (questionnaire, PPD, or chest X-ray), if applicable
+ Current health certificate (per contract or state regulations)
+ Must meet all federal, state, and local requirements
+ Must be at least 18 years of age
